Grant Request

Isolation From Work
If you are applying for a grant because you are self-isolating please upload a copy of your isolation notes for at least 2 weeks.  You can obtain an isolation note from https://111.nhs.uk/isolation-note/

If you are unable to obtain a self-isolation note, we also accept any of the following, based on your circumstances;

·       A signed letter from your employer on a Company Letterhead, that states the length of self-isolation from work

OR

·       An SSP form that states time off work

OR

·       An NHS shielding letter


Please note, that we deal with each application on a case-by-case basis.
